Concentric and eccentric quadriceps limb symmetry indices have an impact on balance and hop performance in patients with ACL reconstruction

Objectives: The aim of the study was to determine the relationship between quadriceps concentric and eccentric strength and functional outcomes of patients with Anterior Cruciate Ligament (ACL) reconstruction.

Methods: Thirty-one professional athletes (age: 28.2 years, body mass index: 27.1 kg/m2, time after surgery: 8.1 months) who had undergone ACL reconstruction surgery included in the study. Concentric and eccentric quadriceps femoris (QF) muscle strength were measured with isokinetic dynamometer at 60/sec. Y balance test (YBT) with anterior, posteromedial and posterolateral reach directions were used for dynamic balance and vertical jump (VJ) and one leg hop (OLH) tests were performed for functional performance. Limb Symmetry Indices (LSI) was calculated for all measurements. Pearson correlation test was used for statistical analysis.

Results: There was a moderate correlation between YBT anterior reach LSI and concentric (p=0.001, r=0.56) and eccentric quadriceps strength LSI (p=0.01, r=0.44). Strong correlation was observed between eccentric quadriceps strength LSI and YBT posteromedial reach distance LSI (p=0.04 and r=0.66). There were weak correlations between quadriceps eccentric LSI and VJ LSI (p=0.03, r=0.36) and quadriceps concentric strength LSI and OLH LSI (p=0.028, r=0.34).

Conclusions: Both eccentric and concentric quadriceps strengths have influence on dynamic balance and hop performance. Therefore, restoring quadriceps strength symmetry is an important factor for enhancing functional performance of the athletes with ACL reconstruction.
